Assumption of Risk: I acknowledge that participation in Lawrence Township Junior Baseball Softball Association (LTJBSA) activities—including but not limited to practices, games, clinics, and the use of facilities—involves inherent risks of serious injury, permanent disability, or death. I voluntarily and freely assume all risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my child’s participation.
Release and Hold Harmless: I hereby release, waive, and discharge LTJBSA, its officers, board members, coaches, volunteers, and the Township of Lawrence, NJ, along with their respective employees and agents (collectively, "Releasees"), from any and all liability, claims, or causes of action for personal injury, property damage, or wrongful death. This release applies whether the injury is caused by the active or passive negligence of the Releasees or otherwise, to the fullest extent permitted by law.
Medical Authorization: In the event of an injury or medical emergency during an LTJBSA activity, I hereby grant permission to LTJBSA representatives to seek and authorize necessary medical treatment for my child if I cannot be reached. I agree to be solely responsible for any costs or expenses associated with such medical treatment.
Acknowledgement of Understanding: I have read this Waiver and Release of Liability, fully understand its terms, and understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this agreement (or clicking "Accept") freely and voluntarily.
